All of the forms below are in PDF format and require Adobe Acrobat Reader. You can download Acrobat Reader free of charge from Adobe's website. 

• Application For Electrical Service - form used to put a location in your name.
• Transferring Membership of Location - form used for permission to transfer the membership of a location
• Contract for Security Lighting - form used to rent a security light from FEM, either Metered or Unmetered
• Removal Release - form used to have an electrical service removed and cease monthly charges. Reconnection would include fees to rebuild the service.
• ACH Authorization Form - form used to authorize payments to be automatically removed from your checking or savings account.
